Worthington Pediatrics is your child's Medical Home
We are happy to inform you that Worthington Pediatrics has been recognized by the National Committee for Quality Assurance as a Patient Centered Medical Home. At Worthington Pediatrics, we have always delivered comprehensive team healthcare and will continue our dedication to providing state of the art care.
What is a Medical Home?
A Medical Home is a team-based approach to healthcare. Together, we will continue to work to coordinate the services your child needs and to provide the best healthcare possible for your child.
What are the changes I can anticipate?
Team Based Care
You will still be able (and encouraged!) to choose your child’s Primary Care Provider (PCP). Your child will continue to have access to our team of other PCPs, nurses, and support staff at Worthington Pediatrics as we work to provide all of your child’s healthcare needs.
As we continue our gradual progression to electronic health records, you will eventually be able to access your health information throught a patient portal.
As always, our clinicians will see your child on the very day he/she has an urgent healthcare need. You will simply call the office at (352) 371-3604 during working hours to schedule a same-day appointment with us.
Routine and follow-up care will be provided in a timely manner and can be arranged by calling our team directly.
We will be monitoring your child’s medical needs and will proactively contact you to schedule follow-up appointments at a convenient time for you.
Your Medical Home Team here at Worthington Pediatrics will continue to help coordinate your follow-up and specialty appointments.
Improved Communication and Access to Information:
You can communicate with a staff member of Worthington Pediatrics anytime during normal working hours by calling (352) 371-3604.
Our new website, implemented in 2014, was designed to help you gain access to the important and update information about your child's healthcare needs. Now that you are here, we invite you to have a look around! Updates are made regularly, so be sure to bookmark this for future use.
How will I contact my Medical Home Team at Worthington Pediatrics?
You can contact us directly to arrange an appointment or for clinical advice by calling (352) 371-3604 Monday-Friday (except holidays) between the hours of 8:00 AM and 5:00 PM.
Outside of normal clinic hours, you may call our office on Saturday mornings from 8:00 AM to 12:00 PM, and your child can be seen/clinical questions will be answered by either our office or that of our call partners.
Pediatric After Hours (PAH) is a non-ER option run by UF Health Shands Hospital and the UF College of Medicine for all pediatric patients who need care after regular hours. PAH is staffed by our own clinicians, and our office will monitor/follow-up with you regarding visits or calls placed to PAH. You must schedule an appointment to be seen at PAH. Call (352) 265-0724 to make an appointment for your child to be seen. Hours and location can be found here: UF Health PAH.
Please continue to call 911 for all emergencies.
